ABSTRACT

Background: Provisional Prosthesis in fixed partial dentures are subjected to Flexure under stress. Selection of appropriate material for fabrication of Provisional is of utmost importance as the Provisional prosthesis has to remain in function till definitive prosthesis is delivered. Material & Methods: Bar type specimens of four different commercially available brands for provisional restorations fabricated according to ADA specification No. 27 and immersed in artificial saliva. The specimens were fractured under 3-point loading test. Results: The flexural strength ranged between 60 to 110 Mpa. BisGMA Auto polymerizing composite resin from Dentsply Caulk shows the highest flexural strength. Conclusion: Within the limitations of this study, the flexural strengths were material specific rather than category one. The BisGMA composite based resin shows significantly higher flexural strength over other materials

ABSTRACT

A cranioplastia para os tratamentos de defeitos ósseos cranianos tem como o seu principal objetivo a reconstrução tridimensional e funcional da calota craniana. As cirurgias assistidas por computador (CAS) vem sendo utilizadas desde os anos 90 de forma eficiente e trazendo melhorias e otimização nas abordagens cirúrgicas craniofaciais reconstrutivas, principalmente em grandes defeitos ósseos. Este relato de caso clínico aborda o planejamento virtual e de tecnologia CAD/CAM na reconstrução craniofacial secundária com a utilização de polimetilmetacrilato (PMMA). Paciente de sexo masculino, 48 anos, apresentava dois defeitos ósseos em região frontal com deiscência da pele para dentro do seio frontal. Foi realizada uma tomografia computadorizada com cortes de 1mm e convertidos em um modelo 3D do osso frontal e no molde do defeito ósseo em tamanho real. Para abordagem dos defeitos ósseos, houve a participação de um neurocirurgião para o tratamento em dura-máter, cranialização do seio frontal e obliteração do ducto naso-frontal, sendo finalizada pela equipe de cirurgia bucomaxilofacial. Após a cirurgia, foi realizado um exame tomográfico sendo observados uma perfeita adaptação entre a prótese e os contornos ósseos e um ótimo contorno anatômico do osso frontal, tornando-se satisfatório ao planejamento cirúrgico inicial. A utilização de um planejamento virtual e do sistema CAD/CAM resultou em uma maior previsibilidade e maior segurança ao procedimento de reconstrução craniofacial além de redução do tempo transoperatório. O material utilizado, o PMMA, apresentou-se como um material de fácil manipulação, baixo custo e com perfeita adaptação aos contornos ósseos.


Cranioplasty for the treatment of cranial bone defects has as its main objective the three-dimensional and functional reconstruction of the skull. Computer-assisted surgeries (CAS) have been used since the 1990s efficiently and bring improvements and optimization in reconstructive craniofacial surgical approaches, especially in large bone defects. This clinical case report addresses virtual planning and CAD/CAM technology in secondary craniofacial reconstruction using polymethylmethacrylate (PMMA). A 48-year-old male patient had two bone defects in the frontal region with skin dehiscence into the frontal sinus. A computed tomography was performed with 1mm slices and converted into a 3D model of the frontal bone and in the mold of the bone defect in real size. To address the bone defects, a neurosurgeon was involved in the treatment of dura mater, cranialization of the frontal sinus, and obliteration of the nasofrontal duct, and was completed by the oral and maxillofacial surgery team. After the surgery, a tomographic exam was performed, and a perfect adaptation between the prosthesis and the bone contours and a great anatomical contour of the frontal bone were observed, making it satisfactory to the initial surgical planning. The use of virtual planning and the CAD/CAM system resulted in greater predictability and greater safety for the craniofacial reconstruction procedure, as well as a reduction in the perioperative time. The material used, PMMA, presented itself as a material of easy manipulation, low cost, and with perfect adaptation to bone contours.Keywords: PMMA, Bone transplantation, Maxillofacial Prosthesis, Cranioplasty, Customized implants.

Objetivo: Este estudo avaliou as propriedades de polimento dos materiais de splint oclusal obtidos usando métodos de fabricação subtrativos e aditivos com os procedimentos de polimento laboratorial (LP) e polimento em consultório (CP). Material e Métodos: As amostras (N=180, n=60 para cada grupo) foram fabricadas usando um dos seguintes métodos: método de fabricação subtrativo (SMM) (M-PM Disc, Merz Dental GmbH), método de fabricação aditivo (AMM) (Freeprint Splint 2.0, DETAX GmbH & Co. KG) e o método de fabricação convencional (CMM) (Promolux HC, Merz Dental GmbH). Seguindo os procedimentos de LP e CP, a rugosidade da superfície dos espécimes foi medida usando um perfilômetro de superfície digital. Um espécime representativo foi selecionado de cada grupo, e uma imagem de microscópio eletrônico de varredura (SEM) foi obtida. Resultados: Tanto o método de fabricação quanto os procedimentos de polimento afetaram significativamente os resultados (P<0,01). Os termos de interação também foram significativos (P<0,001). Conclusão: Com ambos os métodos de polimento, a rugosidade superficial do grupo AMM foi a maior e a do grupo CMM a menor. Embora o procedimento CP tenha sido mais eficaz do que LP com ambos os métodos, a rugosidade da superfície ficou abaixo do limite de 0,2 µm após ambos os procedimentos de polimento testados (AU)


Objective: This study evaluated the polishing properties of the occlusal splint materials obtained using subtractive and additive manufacturing methods with the laboratory-type polishing (LP) and chairside-type polishing (CP) procedures. Material and Methods: Specimens (N=180, n=60 each group) were manufactured using one of the following methods: subtractive manufacturing method (SMM) (M-PM Disc, Merz Dental GmbH), additive manufacturing method (AMM) (Freeprint Splint 2.0, DETAX GmbH & Co. KG), and the conventional manufacturing method (CMM) (Promolux HC, Merz Dental GmbH). Following LP and CP procedures, surface roughness of the specimens was measured using a digital surface profilometer. One representative specimen was selected from each group, and a scanning electron microscope (SEM) image was made. Results: Both the manufacturing method and the polishing procedures significantly affected the results (P<0.01). Interaction terms were also significant (P<0.001). Conclusion: With both polishing methods, surface roughness of the AMM group was the highest and the CMM group the least. Although the CP procedure was more effective than LP with both methods, surface roughness was below the 0.2 µm threshold after both polishing procedures tested. (AU)

ABSTRACT

Abstract The aim of this in vitro study was to compare the long-term effect of overnight use of denture cleansers with different chemical compositions on the color stability of denture base polymers (DBPs). The four DBPs evaluated were PEEK (PK group), thermoinjection-molded polyamide (PA group), auto-polymerized polymethylmethacrylate (PMMA) (AP group), and heat-polymerized resin PMMA (HP group). The cleaning agents used were Corega tablet (CT), Protefix tablet (PT), and 0.5% sodium hypochlorite (NaOCl) solution (SH). Distilled water (DW) served as a control. Forty-eight disc-shaped specimens (10mm × 2mm) were prepared from each DBP and randomly immersed in the different storage media (n=12 in each group). Color measurements of the specimens before and after immersion in the denture cleansers were made using a spectrophotometer (VITA Easyshade V). The color changes were measured and recorded in L* a* b*. After 120 d, the CIEDE2000 formula was used to calculate color changes (ΔE00). For analysis of the data, a multivariate analysis of variance was used (p<0.05). The results of the statistical analysis revealed significant color change differences in the DBPs immersed in the various denture cleansers (p<0.05). Groups PA and PK showed the highest values for all solutions. AP-SH showed the highest ΔE00 value in group AP, no significant difference was found between other solutions. In group HP, there was no statistically significant difference between the average ΔE00 values of all solutions. Long-term daily use of the denture cleansers affected the color stability of all the DBPs, although the level of color change was acceptable. Laboratory and clinical studies on the color stability of PEEK are needed to confirm the results of this study.


Resumen El objetivo de este estudio in vitro fue comparar el efecto a largo plazo del uso nocturno de limpiadores de prótesis con diferentes composiciones químicas sobre la estabilidad del color de los polímeros base de prótesis (DBP). Los cuatro DBP evaluados fueron PEEK (grupo PK), poliamida moldeada por termoinyección (grupo PA), polimetilmetacrilato (PMMA) autopolimerizado (grupo AP) y resina PMMA termopolimerizada (grupo HP). Los agentes de limpieza utilizados fueron Corega (CT), Protefix (PT) y la solución de hipoclorito de sodio (NaOCl) al 0,5% (SH). El agua destilada (DW) sirvió de control. Se prepararon 48 especímenes en forma de disco (10mm × 2mm) de cada DBP y se sumergieron al azar en los diferentes medios de almacenamiento (n=12 en cada grupo). Las mediciones del color de las muestras antes y después de la inmersión en los limpiadores de dentaduras se realizaron con un espectrofotómetro (VITA Easyshade V). Los cambios de color se midieron y registraron en L* a* b*. Después de 120 d, se utilizó la fórmula CIEDE2000 para calcular los cambios de color (ΔE00). Para el análisis de los datos, se utilizó un análisis multivariante de la varianza (p<0,05). Los resultados del análisis estadístico revelaron diferencias significativas en el cambio de color de los DBP sumergidos en los distintos limpiadores de dentaduras (p<0,05). Los grupos PA y PK mostraron los valores más altos para todas las soluciones. AP-SH mostró el valor ΔE00 más alto en el grupo AP, no se encontraron diferencias significativas entre las demás soluciones. En el grupo HP, no hubo diferencias estadísticamente significativas entre los valores medios de ΔE00 de todas las soluciones. El uso diario a largo plazo de los limpiadores de dentaduras afectó a la estabilidad del color de todos los DBP, aunque el nivel de cambio de color fue aceptable. Se necesitan estudios de laboratorio y clínicos sobre la estabilidad del color del PEEK para confirmar los resultados de este estudio.

O objetivo desse estudo foi investigar as propriedades de superfície, estabilidade de cor e propriedades mecânicas de resinas acrílicas para base de dentadura do método convencional, processada por micro-ondas, fresada e impressa 3D diante de diferentes tempos de envelhecimento e imersão em bebidas corantes. Além disso, realizar uma revisão sistemática para avaliar e comparar, através de estudos de ensaios clínicos, próteses totais fabricadas pelo método CAD/CAM com fabricadas convencionalmente, em termos de tempo clínico, manutenção, retenção, estabilidade, eficácia mastigatória, satisfação e qualidade de vida. Um total de 480 amostras redondas (10 × 3,3 ± 0,03 mm) foram confeccionadas para avaliação das propriedades de superfície e estabilidade de cor, divididas entre os quatro grupos principais de resinas e subdivididas em quatro tempos de análise (T0, T1, T2 e T3) e bebidas corantes (água, café, vinho e refrigerante cola), resultando um n = 10 para cada subgrupo. Um total de 160 amostras retangulares (64 × 10 × 3,3 ± 0,03 mm) foram confeccionadas para avaliação das propriedades mecânicas, divididas entre os quatro grupos principais de resinas e subdivididas em quatro tempos de análise (T0, T1, T2 e T3), resultando um n = 10 para cada subgrupo. A revisão sistemática seguiu os Itens de Relatório Preferenciais para Revisões Sistemáticas e Meta-Análises (PRISMA) e foi registrada no Registro Prospectivo Internacional de Revisões Sistemáticas (PROSPERO) (CRD42022296907). A busca eletrônica foi realizada nas bases de dados PubMed/MEDLINE, Web of Science e Scopus até dezembro de 2021. A questão formulada para população, intervenção, comparação e resultado (PICO) foi "Pacientes reabilitados com próteses totais com tecnologia CAD/CAM apresentam parâmetros clínicos semelhantes aos pacientes reabilitados com próteses totais convencionais?". A resina fresada apresentou os melhores resultados das propriedades de superfície diante do efeito do envelhecimento, enquanto que a resina impressa 3D apresentou resultados semelhantes as resinas tradicionais. A resina de Impressão 3D apresentou não ter propriedades ópticas adequadas diante do efeito do envelhecimento, enquanto que a resina fresada apresentou as melhores propriedades. As bebidas corantes não alteraram as propriedades de superfície das resinas, entretanto afetaram suas propriedades ópticas, principalmente a resina de impressão 3D e a processada por micro-ondas. A resina de dentadura fresada CAD/CAM apresentou propriedades mecânicas semelhantes as resinas tradicionais, enquanto que a impressa 3D não apresentou propriedades mecânicas adequadas para uso clínico à longo prazo. As próteses do método de fresagem apresentaram melhor desempenho clínico que as próteses convencionais, em termos de qualidade de vida, satisfação, retenção, estabilidade, tempo clínico e custo, enquanto que as de impressão 3D não apresentaram diferenças. Ambos os métodos do CAD/CAM apresentaram não haver diferenças da eficácia mastigatória em relação as próteses convencionais(AU)


The objective of this study was to investigate the surface properties, color stability and mechanical properties of acrylic resins for denture base of the conventional method, processed by microwave, milled and 3D printed in face of different aging times and immersion in coloring drinks. In addition, carry out a systematic review to evaluate and compare, through clinical trial studies, complete dentures manufactured by the CAD/CAM method with those manufactured conventionally, in terms of clinical time, maintenance, retention, stability, masticatory efficiency, satisfaction and quality of life. A total of 480 round samples (10 × 3.3 ± 0.03 mm) were made to evaluate surface properties and color stability, divided among the four main groups of resins and subdivided into four analysis times (T0, T1, T2 and T3) and coloring drinks (water, coffee, wine and cola), resulting in n = 10 for each subgroup. A total of 160 rectangular samples (64 × 10 × 3.3 ± 0.03 mm) were prepared to evaluate the mechanical properties, divided among the four main groups of resins and subdivided into four analysis times (T0, T1, T2 and T3), resulting in n = 10 for each subgroup. The systematic review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and was registered in the International Prospective Registry of Systematic Reviews (PROSPERO) (CRD42022296907). The electronic search was performed in the PubMed/MEDLINE, Web of Science and Scopus databases until December 2021. The question formulated for population, intervention, comparison and result (PICO) was "Patients rehabilitated with complete dentures with CAD/CAM technology present clinical parameters similar to patients rehabilitated with conventional complete dentures?". The milled resin presented the best results of surface properties in the face of aging effect, while the 3D printed resin presented similar results to traditional resins. The 3D Printing resin did not have adequate optical properties due to the aging effect, while the milled resin showed the best properties. Coloring drinks do not change the surface properties of resins, but optical printing solutions, mainly 3D printing resin and microwave-processed resin. The CAD/CAM milled denture resin presented mechanical properties similar to traditional resins, while the 3D printed denture resin did not present adequate mechanical properties for long-term clinical use. The milling method prostheses showed better clinical performance than conventional prostheses, in terms of quality of life, satisfaction, retention, stability, clinical time and cost, while the 3D printing prostheses showed no differences. Both CAD/CAM methods showed no differences in masticatory efficiency in relation to conventional prostheses(AU)

ABSTRACT

Objective: This study aimed to evaluate the clinical performance of two inlay-retained bridge designs (proximal shaped and inlay shaped) in single missing posterior teeth cases. Material and Methods: A total of 70 cases with missing single posterior teeth were included in this study and divided into two groups with a 1:1 allocation ratio (n = 35 for each group). Group 1 (the control group): this group received an inlay retained bridge with inlay design on both abutments. Group 2 (the intervention group): this group received an inlay retained bridge with a proximal box on both abutments. PMMA resin (YAMAHACHI PMMA) was used for the try-in stage and monolithic zirconia (Katana, Kuraray) was used for the final restorations. The restoration surfaces were treated using sandblasting and Z-prime S (Bisco) and the cementation was done by using self-adhesive resin cement (Bisco). Fracture, marginal adaptation, postoperative sensitivity, caries, and gingivitis were assessed using the modified United States Public Health Service for restoration clinical assessments (MUSPHS standards) over 12 months of follow-up. Results: The results show there was no significant difference between the two groups. Kaplan-Meier survival curve was constructed to calculate the mean survival estimates of the two groups and we found that the two groups were clinically successful during a one-year follow-up. Conclusion: Both designs of inlay retained fixed dental prostheses revealed successful clinical performance in terms of Fracture, marginal adaptation, postoperative sensitivity, caries, and gingivitis (AU)


Objetivo: O objetivo deste estudo foi avaliar o desempenho clínico de dois modelos de ponte fixa por inlay (formato proximal e formato inlay) em casos de dentes posteriores perdidos. Material e Métodos: Um total de 70 casos com perda de um único dente posterior foi incluído neste estudo e dividido em dois grupos com uma razão de alocação de 1: 1 (n = 35 para cada grupo). Grupo 1 (grupo controle): Este grupo recebeu uma ponte fixa por inlay com deseho de inlay em ambos os pilares. Grupo 2 (grupo intervenção): Este grupo recebeu uma ponte fixa inlay com uma caixa proximal em ambos os pilares. Resina de PMMA (YAMAHACHI PMMA) foi usada para a etapa de try-in e a zircônia monolítica (Katana, Kuraray) foi utilizada para as restaurações finais. As superfícies das restaurações foram jateadas com Z-prime S (Bisco) e a cimentação realizada com cimento resinoso autoadesivo (Bisco). Fratura, adaptação marginal, sensibilidade pós-operatória, cárie e gengivite foram avaliadas usando o Serviço de Saúde Pública dos Estados Unidos modificado para avaliações clínicas de restauração (padrões MUSPHS) ao longo de 12 meses de acompanhamento. Resultados: Os resultados mostraram que não houve diferença significativa entre os dois grupos. A curva de sobrevida de Kaplan-Meier foi construída para calcular as estimativas de sobrevida média dos dois grupos e foi concluído que os dois grupos foram clinicamente bem-sucedidos durante um acompanhamento de um ano. Conclusão: Ambos os modelos de próteses dentárias fixas de inlay revelaram desempenho clínico bem-sucedido em termos de fratura, adaptação marginal, sensibilidade pós-operatória, cárie e gengivite(AU)

ABSTRACT

Pacientes com históricos de defeitos ósseos provocados por infecções, malformação congênita, neoplasias, deformação por iatrogenia, radioterapia e trauma buscam contornos faciais mais harmônicos através da reabilitação cirúrgica. Para facilitar a reconstrução maxilofacial dois grupos de materiais podem ser utilizados, os enxertos ósseos e os materiais aloplásticos. O objetivo é relatar um caso incomum de infecção e exposição de material utilizado para enxertia a base de polimetilmetacrilato, bem como a sua posterior reabordagem cirúrgica. Embora o Polimetilmetacrilato aparente ser seguro, ele exibe complicações diversas em função da imunologia do hospedeiro, que poderá reagir de diferentes formas. Desse modo, faz-se necessário ressaltar a importância da prevenção e proservação de cada caso de forma individualizada(AU)


Patients with a history of bone defects caused by infections, congenital malformation, neoplasms, iatrogenic deformation, radiotherapy and trauma seek more harmonious facial contours through surgical rehabilitation. To facilitate maxillofacial reconstruction, two groups of materials can be used, bone grafts and alloplastic materials. The objective is to report an unusual case of infection and exposure of material used for grafting with polymethylmethacrylate, as well as its subsequent surgical approach. Although Polymethylmethacrylate appears to be safe, it exhibits different complications depending on the host's immunology, which may react in different ways. Thus, it is necessary to emphasize the importance of preventing and preserving each case individually(AU)

ABSTRACT

A aplicação de selantes de superfície poderá ajudar na diminuição da degradação superficial dos blocos de PMMA para CAD/CAM frente aos desafios de absorção de água, variação de temperatura e desafio erosivo causada pelo suco gástrico (frequente em pacientes com refluxo gastroesofágico (DRGE) ou bulimia. Contudo, poucos estudos na área odontológica foram realizados com essas variáveis, sendo necessário o aprofundamento sobre o assunto. Diante disso, o objetivo deste estudo foi avaliar o impacto de diferentes selantes de superfície (Biscovel LV e Palaseal) na preservação das propriedades mecânicas de polímero de PMMA para CAD/CAM, submetidos a termociclagem (T1 - 5.000 ciclos, de 5 e 55oC ; T2 ­ 10.000 ciclos, de 5 e 55oC) e desafio erosivo (DE1 - imersão em HCl 5% (pH=2,0) durante 45 horas e DE2- 91 horas), por meio da rugosidade de superfície (Ra), microdureza Knoop (MK), resistência flexural (RF) e módulo de elasticidade (ME). Foram confeccionados 90 espécimes quadrados (10x10x3 mm) para as análises de Ra e MK e 180 espécimes retangulares (25x2x2mm) para as análises de RF e ME. Os espécimes foram divididos em grupos (n=10) de acordo com o tratamento de superfície e envelhecimento. Os polimentos realizados foram: PA - pontas de borracha Astropol; PA+SB - polimento com PA + aplicação de selante de superfície Biscover LV e PA+SP - polimento com PA + aplicação de selante de superfície Palaseal. As análises foram realizadas após os tratamentos de superfície e após os períodos dos envelhecimentos. Os dados obtidos foram submetidos ao ANOVA (2 fatores), teste Bonferroni (α=0,05) e a análise de regressão linear simples. Na análise de Ra, em todos os períodos de termociclagem o grupo PA obteve os maiores valores de Ra e PA+SP os menores, com diferença significativa entre eles (p< 0.05). De acordo com a análise de regressão linear, todos os grupos apresentaram tendência linear crescente. Já após DE1 e DE2 os três grupos diferiram estatisticamente entre si (p< 0.05), sendo classificados em ordem crescente de rugosidade em PA+SP < PA+SB< PA. O grupo PA+SP manteve os valores de Ra no decorrer dos períodos de análises. Para RF, ocorreu diminuição significativa dos valores de RF do grupo PA após T1 e T2, diferindo estatisticamente dos demais grupos (p< 0.05). O desafio erosivo diminuiu de forma significante (p< 0.05) os valores de RF dos grupos PA e PA+SP após DE1 e, do grupo PA+SB após DE2. Não houve diferenças significativas nas análises de MK e ME, independente dos tratamentos de superfície e envelhecimentos. Conclui-se que os selantes de superfície foram capazes de preservar a rugosidade de superfície e resistência flexural de polímero de PMMA para CAD/CAM submetidos a termociclagem ou desafio erosivo(AU)


The application of surface sealants can help to reduce the surface degradation of PMMA blocks for CAD/CAM in the face of water absorption challenges, temperature variation and erosive challenge caused by gastric juice (common in patients with gastroesophageal reflux (DRGE) or bulimia. However, few studies in the field of dentistry have been conducted with these variables, requiring a more in-depth look at the subject. Therefore, the aim of this study was to evaluate the impact of different surface sealants (Biscovel LV and Palaseal) on the preservation of mechanical properties of PMMA polymer for CAD/CAM, subjected to thermocycling (T1 - 5,000 cycles, from 5 and 55oC; T2 - 10,000 cycles, from 5 to 55oC) and erosive challenge (DE1 - immersion in 5% HCl (pH=2.0) for 45 hours and DE2-91 hours), through surface roughness (Ra), Knoop microhardness (MK), flexural strength (RF) and modulus of elasticity (ME). The Ra and MK analyzes and 180 rectangular specimens (25x2x2mm) for the RF and ME analyses. Specimens were divided into groups (n=10) according to surface treatment and aging. The polishes performed were: PA - Astropol rubber tips; PA+SB - PA polish + application of Biscover LV surface sealant and PA+SP - polish with PA + application of Palaseal surface sealant. Analyzes were performed after surface treatments and after aging periods. The data obtained were submitted to ANOVA (2 factors), Bonferroni test (α=0.05) and simple linear regression analysis. In the analysis of Ra, in all thermocycling periods, the PA group had the highest values of Ra and PA+SP the lowest, with a significant difference between them (p< 0.05). According to the linear regression analysis, all groups showed an increasing linear trend. After DE1 and DE2, the three groups differed statistically from each other (p< 0.05), being classified in ascending order of roughness in PA+SP < PA+SB < PA. The PA+SP group maintained the Ra values throughout the analysis periods. For RF, there was a significant decrease in the RF values of the PA group after T1 and T2, differing statistically from the others groups (p< 0.05). The erosive challenge significantly decreased (p< 0.05) the RF values of the PA and PA+SP groups after DE1 and of the PA+SB group after DE2. There were no significant differences in the MK and ME analyses, regardless of surface treatments and aging. It was concluded that the surface sealants were able to preserve the surface roughness and flexural strength of PMMA polymer for CAD/CAM submitted to thermocycling or erosive challenge(AU)

Abstract Denture adhesives need complete removal due to their frequent replacement. Objective Our study investigates the removal of denture adhesives from denture base materials, using different methods. Methodology PMMA and Polyamide denture base materials were used to fabricate 120 samples (15×15×1.5mm). One side of the samples was left as processed and the other polished with a usual procedure, hydrated for 24 h, dried, and weighted. They received 0.2 g of three adhesive creams on their unpolished surface (Corega, Olivafix, Fittydent), pressed on polysulfide material, stored under 37°C and 95% rel. humidity for 1 h and 60 of them, following their separation from polysulfide base, brushed under running water, whereas the rest inserted in a cleanser bath (Fittydent Super) for 5 min. The samples were dried and inserted in the oven (37°C) for additional 10 min and weighted again. Roughness tests of denture materials and light microscopy of adhesives creams were also used to evaluate the materials. Time lapse images of spayed with water adhesives on PMMA base were also taken to evaluate the volumetric changes of adhesives. Weight data before and after adhesive removal, indicating the amount of remaining adhesive, were statistically analyzed using Welch's ANOVA and Games-Howell multiple comparisons tests at α=0.05 level of significance. Results Roughness of Polyamide was higher than PMMA and Fittydent showed greater volumetric changes than the others. Significant differences (p<0.05), were found between PMMA and Polyamide bases, between Olivafix and Fittydent adhesives, and between brushing and cleansing methods but only for PMMA-Olivafix combination. Conclusions Adhesives showed a stronger adherence to PMMA surface, and Fittydent was the most difficult to be removed. Removal methods were not effective for all adhesives or denture base materials. These indicate that removal methods, adhesive type and denture base material are all playing a significant role in the removal of adhesives from denture surfaces.

Introducción: los defectos del cráneo y las anomalías del hueso craneofacial que requie-ren reconstrucción son comunes en una variedad de procedimientos neuroquirúrgicos. Después de una craniectomía o de fracturas craneofaciales posteriores a traumatismos cráneoencefálicos, los pacientes pueden desarrollar defectos cosméticos importantes. Algunos de estos son la depresión de la piel y un defecto de hundimiento que lleva a una apariencia asimétrica de la cabeza, sin dejar de lado las repercusiones físicas, neu-rológicas y psicológicas que estas lesiones conllevan. La reconstrucción craneofacial y la craneoplastía tienen una larga historia, pero las nuevas técnicas quirúrgicas y una multitud de opciones de materiales han impulsado recientemente el avance en esta área. Los implantes de polimetilmetacrilato (PMMA) han demostrado ser estables, bio-compatibles, no conductores, radiotransparentes y de bajo costo. Es así que se pueden colocar y modificar fácilmente, con lo que se elimina la morbilidad del sitio donante. Presentación del caso: en este artículo presentamos un caso de craneoplastía de defec-to frontal, posterior a traumatismo, cuya reconstrucción fue realizada mediante una prótesis de polimetilmetacrilato (PMMA) en el Hospital General Xoco de la Cuidad de México. Conclusión: se reporta la reducción del tiempo quirúrgico, además de un costo de la prótesis accesible para el paciente; de esta manera, se obtuvieron resultados sa-tisfactorios y mejoras en el contorno estético facial, en tanto se permitió cobertura y protección para el tejido encefálico.


Introduction: Skull defects and craniofacial bone abnormalities that require reconstruction are common in a variety of neurosurgical procedures. After craniectomy or craniofacial fractures following cranioencephalic trauma, patients can develop important cosmetic defects, such as depression of the skin and a sunken flap that can lead to an asymmetrical appearance of the head, without neglecting the physical and psychological repercussions. neurological that these injuries carry. Craniofacial reconstruction and cranioplasty have a long history, but new surgical techniques and a multitude of material options have recently fueled progress in this area. Polymethylmethacrylate (PMMA) implants have proven to be stable, biocompatible, non-conductive, radiolucent, and inexpensive. They can be easily placed and modified and the morbidity of the donor site is eliminated. Clinical case: In this article, we present a case of frontal defect cranioplasty after trauma performed using a polymethylmethacrylate (PMMA) prosthesis at the General Hospital Xoco in Mexico City. Conslusion: The surgical time could be reduced, in addition to the cost of the prosthesis. It was accessible to the patient and obtained satisfactory results such as improvements in the facial aesthetic contour and obtained coverage and protection for the brain tissue.

ABSTRACT

Objetivou-se com este trabalho analisar a resistência de placas ósseas de acrílico, confeccionadas com cortadora a laser CO2, quando submetidas às forças que agem sobre o foco de fratura. Foram utilizados 40 fêmures de ovinos (Ovis aries) e confeccionadas 40 placas de acrílico, os quais foram avaliados em grupos: grupo OP - foram avaliados 20 ossos fraturados reduzidos com 20 placas de acrílico; grupo PS - foram avaliadas 20 placas de acrílico; e grupo OS - foram avaliados 20 ossos íntegros e frescos. Todos foram submetidos a ensaios mecânicos de compressão, flexão, torção e tração, em máquina de ensaios universal EMIC® 10000. Os testes eram interrompidos quando havia fratura do corpo de prova. O grupo PS suportou carga de compressão significativamente superior aos demais grupos OP e OS (13.255±290N, 4.932±827N e 8.681±303N, respectivamente). Já em relação à prova de flexão, o grupo OS foi significativamente mais resistente, com média de 2.698±305N, em relação aos grupos OP e PS, que obtiveram 1.315±92N e 1.537±37N, respectivamente. Conclui-se que as placas de acrílico usinadas a laser apresentam resistência mecânica inferior ao fêmur de ovinos, entretanto, devido à resistência exibida nos testes, demonstraram potencial para serem implantadas em ossos com menor demanda de carga.(AU)


The objective of this work was to analyze the resistance of acrylic bone plates, made with CO 2 laser cutter, when subjected to the forces acting on the fracture focus. Forty ovine femurs (Ovis aries) were used and 40 acrylic plates were prepared and evaluated in groups: In the OP Group, 20 fractured bones reduced with 20 acrylic plates were evaluated; in the PS Group, 20 acrylic plates were evaluated; and in the OS Group 20 intact and fresh bones were evaluated. All were submitted to mechanical tests of compression, bending, twisting and traction in an EMIC® 10000 universal testing machine. The tests were interrupted when the body of proof was fractured. The PS group supported a significantly higher compression load than the other OP and OS groups (13,255±290 N, 4,932±827 N and 8,681±303 N, respectively). Regarding the flexural test, the OS group was significantly more resistant, with a mean of 2,698±305 N, compared to the groups OP and PS that obtained 1,315±92 N and 1,537±37 N respectively. It is concluded that the laser-machined acrylic plates have inferior mechanical resistance to the femur of the ovine, however, due to the presented resistance, it shows potential to be implanted in bones with lower load demand.(AU)

Purpose: The purpose of the study was to evaluate the effect of titanium dioxide and zirconia nanoparticles on transverse strength of heat cure PMMA resin routinely used for complete denture fabrication.Methods: One hundred samples of PMMA resin were made and divided into five groups (20 samples for each group). The test specimens were divided into five groups depending on the concentration of reinforcing nanoparticles as Group 1,2,3,4 and 5; Group 1: PMMA unreinforced (control group), Group 2: PMMA reinforced with 2.5% nanozirconia, Group 3: PMMA reinforced with 5% nanozirconia, Group 4: PMMA reinforced with 2.5% titanium dioxide nanoparticles, and Original ResearchArticle Group 5: PMMA reinforced with 5% titanium dioxide nanoparticles. Universal testing machine was used to conduct a three-point bending test and evaluate the transverse strength of samples. Comparison of mean transverse strength for various groups was carried out by employing one‑way analysis of variance and Bonferroni post hoc tests.Results:The highest and lowest mean transverse strength were observed in Group 3 and 1, respectively. Bonferroni post hoc test showed increase in transverse strength after reinforcement to be statistically significant between all the groups (P = .05) except between the samples of group G1 and G5 and G2 and G3.Conclusion: Addition of nanoparticles in all concentrations significantly increased transverse strength of heat cure PMMA resin as compared to control group. The best result was obtained after adding 5% of nanozirconia particles to the conventional heat polymerized acrylic resin

ABSTRACT

Mechanical properties (impact strength, compressive strength and flexural strength) have been studied for (Polymethyl-methacrylate (PMMA)) polymer reinforced with nano (TiO2+ZnO) (0 v%, 1 v%, 2 v%, 3 v%, 4 v% and 5 v%). Ultrasonic dispersion technique was used to prepare the nanocomposites samples, follow with cold casting technique using Teflon molds standard conditions. Charpy impact instrument was used to measure the impact strength, the compressive strength of each sample which was tested in newton units using digital compression tester (TinusOlsen: H50KT, England), while flexural strength can be found from mathematical formulas which depend on the impact strength, compressive strength and flexural strength. Results show that values increase in 1 v%, then decrease alternately by (2 v%, 3 v%, 4 v%, and 5 v%), while 0 v% is the less value of volume fraction of fillers. Young modules values increase alternately by the volume fraction of fillers until 2 v%, while if it is higher and lower than 2 v% it decreases the modulus of elasticity.

ABSTRACT Polymethylmetacrylte (PMMA) is used in the fields of dentistry and biomedicine as a constituent of bone cements. Hydroxyapatite (HAp) is a bioceramic produced naturally in the bones. PMMA and HAp are fundamental constituents in the preparation of bone cements. Bisphosphonates have also been used as radiopharmaceutical in dental implants and nuclear medicine, or as palliative systemic treatment for pain reduction in bone metastasis. Vertebroplasty and kyphoplasty are bone cement-based techniques used in orthopedics, being minimally invasive procedures with low risks of infections, applied in osteoporosis and high-impact fractures. Recently, Núcleo de Radiações Ionizantes da Universidade Federal de Minas Gerais proposed a synthetic composite of M-HAp with a metallic nuclide M. After irradiation, M-HAp was added to PMMA, compounding a radioactive bone cement that can recover bone body stabilization, pasting microfractures and recomposing the anatomy and functionality of the affected parts by the compression of bone metastases, with possible pain reduction through quick radiation-induced decompression. Computational dosimetric models, and the synthesis and characterization of bioceramics that incorporate Re-188, Ho-166, or Sm-153 have demonstrated the benefits of these biometrics as promising alternative therapies, mainly from their ability to maintain the ionization in the bone structure, thereby sparing the spinal cord. RESUMO Polymethylmetacrylte (PMMA) é um composto utilizado na indústria e desde os anos 80's também tem sido empregado nas áreas odontológica e biomédica como constituinte de cimentos ósseos. A hydroxyapatite (HAp) é uma biocerâmica produzida naturalmente nos ossos. Esses dois componentes são constituintes fundamentais no preparo de cimentos ósseos. A síntese artificial de HAp pode ser feita pelo método sol-gel. Bifosfanatos tem também sido utilizado na odontologia em implantes dentários e na medicina nuclear, como radiofármaco ou no tratamento paliativo sistêmico de redução de dor das metástasis ósseas com 153 Sm-EDTPM. A Vertebroplastia e Cifoplastia são técnicas empregadas na ortopedia utilizando o cimento ósseo, sendo procedimentos minimamente invasivos de baixo risco de infeções, aplicadas em osteoporose e fraturas de alto impacto. Recentemente, no Grupo de Pesquisa NRI - Nucleo de Radiações Ionizantes/UFMG, foi proposto um compósito de M-HAp com nuclideo metálico M incorporado na matriz, que após ativado é adicionado ao PMMA constituíndo um cimento ósseo radioativo que pode recuperar a estabilização o corpo ósseo, colando microfraturas, recompondo a anatômia e funcionalidade de peças afetadas pela compressão das metástases ósseas com possível redução de dor pela rápida descompressão induzida pela radiação. Modelos dosimétricos computacionais, sintese e caracterização destas biocerâmicas incorporando Re-188, Ho-166 ou Sm-153 tem indicado benefícios radioterapêuticos promissores, podendo se tornar uma alternativa para radioterapias convencionais, principalmente por conter a dose absorvida na estrutura óssea, por exemplo no corpo da vertebra poupando a medula espinhal. ABSTRACT

Background: Posterior capsular opacification (PCO) which is also known as “after cataract” or “secondary cataract”, is the most common complication of cataract surgery, with an incidence of 20-50%. The current study was conducted in a tertiary hospital of Odisha with an objective to find out the determinants of PCO among patients with defective vision attending the outdoor patient department of Ophthalmology.Methods: A hospital based descriptive study was conducted among the patients attending the ophthalmology out patient department of a tertiary hospital of Odisha. The detail history regarding the type of surgical procedure used for cataract extraction and the type of Intra Ocular Lens (IOL) implanted, duration of post-operative period was collected from the available documents and ophthalmic examination of the participants.Results: In the present study, 184 participants were included and examined. Fifty percent of the participants had undergone conventional extra capsular cataract extraction procedure. In 86.95% participants, the IOL used was Poly Methyl Methacrylate lens (PMMA). In 26.08% of the participants the development of PCO was within 12 to 36 months of cataract surgery. The average duration of PCO development recorded for participants <20 years was 3 months.Conclusions: Most of the participants included in the study with PCO had undergone conventional ECCE surgery, implanted PMMA lens, IOL with round edge and had a duration of 12-36 months between cataract surgery and PCO development. The average duration of PCO development is less among younger participants which gradually increases with increase in age.

Abstract Candida albicans is the main causative pathogen of denture stomatitis, which affects many complete denture patients. Objective: To evaluate the effect of different concentrations of nanodiamonds (NDs) added to polymethyl methacrylate (PMMA) denture base material on Candida albicans adhesion as well as on surface roughness and contact angle. Methodology: Acrylic resin specimens sized 10×10×3 mm3 were prepared and divided into four groups (n=30) according to ND concentration (0%, 0.5%, 1%, 1.5% by wt). Surface roughness was measured with a profilometer, and the contact angle with a goniometer. The effect of NDs on Candida albicans adhesion was evaluated using two methods: 1) slide count and 2) direct culture test. Analysis of variance (ANOVA) and Tukey's post hoc test were used in the statistical analyses. Results: Addition of NDs decreased the Candida albicans count significantly more than in the control group (p<0.05), with a lowest of 1% NDs. Addition of NDs also significantly decreased the surface roughness (p<0.05), but the contact angle remained the same. Incorporation of NDs into the PMMA denture base material effectively reduced Candida albicans adhesion and decreased surface roughness. Conclusion: PMMA/NDs composites could be valuable in the prevention of denture stomatitis, which is considered one of the most common clinical problems among removable denture wearers.

ABSTRACT A cytotoxicity study is performed on a poly(methyl methacrylate) polymer (PMMA) to be used for the fabrication of bone tissue by Rapid Prototyping (RP). The solution polymerization is conducted in a pilot plant reactor using more appropriated reagents in consideration of the medical application. Moreover, the polymer is efficiently handled to avoid the side effect of the monomer, reducing the concentration of this specie to 287,731 µg MMA/kg PMMA. The cytotoxicity of the polymer is determined through growth monitoring, adherence and morphology of L-929 cells. Additionally, MTT and LIVE/DEAD tests are performed. The results showed continuous and progressive growth of the cells on the surface of the specimens. Moreover, the material did not influence on the viability of mesenchymal cells and inverted fluorescence microscopy images showed a polyanionic dye calcein well retained in the cells in contact with the PMMA as well as the negative control after 72 hours. Thus, the polymer was efficiently synthesized and handled for the expected demands.

PURPOSE: The aim of this preliminary study was to investigate, for the first time, the effects of addition of titania nanotubes (n-TiO2) to poly methyl methacrylate (PMMA) on mechanical properties of PMMA denture base. MATERIALS AND METHODS: TiO2 nanotubes were prepared using alkaline hydrothermal process. Obtained nanotubes were assessed using FESEM-EDX, XRD, and FT-IR. For 3 experiments of this study (fracture toughness, three-point bending flexural strength, and Vickers microhardness), 135 specimens were prepared according to ISO 20795-1:2013 (n of each experiment=45). For each experiment, PMMA was mixed with 0% (control), 2.5 wt%, and 5 wt% nanotubes. From each TiO2:PMMA ratio, 15 specimens were fabricated for each experiment. Effects of n-TiO2 addition on 3 mechanical properties were assessed using Pearson, ANOVA, and Tukey tests. RESULTS: SEM images of n-TiO2 exhibited the presence of elongated tubular structures. The XRD pattern of synthesized n-TiO2 represented the anatase crystal phase of TiO2. Moderate to very strong significant positive correlations were observed between the concentration of n-TiO2 and each of the 3 physicomechanical properties of PMMA (Pearson's P value ≤.001, correlation coefficient ranging between 0.5 and 0.9). Flexural strength and hardness values of specimens modified with both 2.5 and 5 wt% n-TiO2 were significantly higher than those of control (P≤.001). Fracture toughness of samples reinforced with 5 wt% n-TiO2 (but not those of 2.5% n-TiO2) was higher than control (P=.002). CONCLUSION: Titania nanotubes were successfully introduced for the first time as a means of enhancing the hardness, flexural strength, and fracture toughness of denture base PMMA.

Introdução: A craniectomia descompressiva é uma cirurgia indicada no tratamento da hipertensão intracraniana em situações graves de traumas cranioencefálicos. Esta cirurgia confere uma aparência bizarra ao paciente, como se "parte da cabeça" houvesse sido retirada. Após a regressão do edema cerebral e quando o paciente reunir boas condições clínicas, a reconstrução craniana está indicada. A reconstrução da calota craniana poderá ser realizada com osso autólogo ou com materiais aloplásticos. Este estudo objetiva apresentar a experiência do autor com reconstruções de calota craniana utilizando próteses customizadas de PMMA. Métodos: Foram selecionados 14 pacientes submetidos à craniectomia descompressiva que, após serem liberados clinicamente pela Neurocirurgia, realizaram a reconstrução da calota craniana com próteses de PMMA customizadas por prototipagem entre os anos de 2014 e 2015 e com, no mínimo, 6 meses de pós-operatório. Sinais e sintomas de síndrome do Trefinado, tomografia computadorizada e aparência estética dos pacientes foram analisadas no pré e no 6º mês pós-operatório. Resultados: Todos os pacientes apresentaram melhora sintomatológica, melhora estética e expansão cerebral após a cirurgia. Conclusão: A reconstrução da calota craniana com prótese customizada de PMMA promoveu a melhora dos sinais e sintomas e da aparência estética de todos os 14 pacientes operados. A utilização de protótipos para customizar próteses cranianas facilitou a técnica operatória e possibilitou a recuperação de um contorno craniano muito próximo da normalidade.


Introduction: Decompressive craniectomy is indicated for the treatment of intracranial hypertension in cases of serious traumatic brain injury. This surgery results in a bizarre appearance, as if "part of the head" had been. After regression of cerebral edema and when the patient is in good clinical condition, the reconstruction of the skull is indicated. Reconstruction of the skull can be performed with autologous bone or with alloplastic materials. This study sought to present the experience of the author with skull reconstructions using custom PMMA prostheses. Methods: In between 2014 and 2015, fourteen patients with previous (longer than 6 months) decompressive craniectomies were selected after Neurosurgery medical clearance and underwent skull reconstruction with customized PMMA prototyped prostheses. Signs and symptoms of syndrome of the trephined, computed tomography, and aesthetic appearance of the patients were analyzed preoperatively and at 6 months after reconstruction. Results: All patients presented with improved symptomatology, aesthetic improvement and expansion of the brain after surgery. Conclusion: Reconstruction of the skull with customized prototyped PMMA prostheses improved the signs and symptoms and the aesthetic appearance in all 14 patients of this series. The use of prototypes to customize cranial prostheses facilitates the operative technique and enables patients to develop a nearly normal cranial contour.

ABSTRACT

Objective To investigate the effect of metformin on the osteogenic differentiation of human mesenchymal stem cells exposed to PMMA particles. Methods Human placental mesenchymal stem cells were iso-lated and cultured in vitro. The effect of metformin with different concentrations on cell viability was determined by CCK8 assay. The effect of metformin on the mRNA expression of osteogenic genes was detected by using real-time RT-PCR. Calcified nodules were stained by alizarin S. The effect of metformin on the expression of eNOS was also detected by using real-time RT-PCR. Results PMMA particles could inhibit the viability of mesenchymal stem cells. Metformin(0.05 mmol/L)could promote the viability of mesenchymal stem cells exposed to PMMA particles. Metformin(0.05 mmol/L)could increase the expression of osteogenic genes,including OCN,RNUX2,and ALP, in human mesenchymal stem cells exposed to PMMA particles. The calcium deposit was also increased after metfor-min treatment. Results of real-time RT-PCR showed that metformin could increase the expression of eNOS in human mesenchymal stem cells exposed to PMMA particles. Conclusions Metformin can increase the osteogenic differentiation of human mesenchymal stem cells exposed to PMMA particles,partially by inducing eNOS expression.
